The basic principle of equity in health care means that health care should be distributed according to need regardless of ability to pay. The extent to which a population gains the services they need to achieve the best possible outcome is determined by accessibility. A barrier to access is indicated when utilization does not occur in the presence of need. This study aims: 1) To evaluate the impact of health care privatization to health care access of the poor. 2) To observe the impact of organizational reform to the provision of basic health care for the poor. 3) To evaluate whether the protection mechanism successfully ensures access for the poor. 4) To identify local initiatives that successfully ensures access for the poor under resource constraints in the context of post decentralization.

This is an observational case study using a qualitative and quantitative approach. The main unit analysis of the study is the district, which are the Tabanan and Jembrana districts in Bali province. Data collection methods included document reviews, community surveys and in-depth interviews.
